HOME Care provider (HCP)
Our Enhanced Home Care, Family support program places individuals with disabilities in a comfortable home environment with a caring and experienced individual, couple, or family. These individuals become a part of the host family and receive support to pursue their personal goals and successfully live in their community. We carefully match clients with home care providers based on their needs, interests, preferences, and goals. This allows clients to have their own room with access to all basic amenities such as cable, phone, and internet, and to live in a supportive and nurturing environment.
Respite program
Our Respite Services provide temporary care for primary caregivers, so they can take a break while maintaining the same level of care for the individual.
Day program
Our Day Program is designed to help clients pursue their personal goals and integrate into the community. The daily schedule is tailored to each individual's preferences and reviewed with them daily. Support staff help clients transition through activities by providing positive verbal reinforcement and cues. Clients are supported in curriculum-based, skill building programs such as seeking employment, healthy lifestyle choices, vocational skills, and safety skills. Socialization, personal expression through the arts, and participation in community activities are also emphasized in the program.
